A Utah dance mom gave her 11-year-old daughter a cocktail of prescription medications and penned a chilling confession note before shooting the girl and herself in a grisly murder-suicide, new autopsy reports reveal.

Tawnia McGeehan, 34, and her daughter, Addilyn Smith, were found dead from gunshot wounds after police were called by the tween’s father to perform a welfare check at the Rio Hotel and Casino in Las Vegas, where the pair had been traveling for Smith’s cheer competition in February.

Immediately upon entering their hotel room, cops encountered the first of several deranged, handwritten suicide notes left behind by the killer mother.

“Call 911 before walking in. Two dead bodies and one is an 11-year-old,” McGeehan had written on a shred of paper taped to a chair that was partially blocking the entrance to the room, according to the Clark County Coroner’s investigative report obtained by The Post.

Toxicology results included in the newly released reports also show that the mother-daughter duo had ingested a myriad of drugs before the gunfire erupted.

Substances including quetiapine, an antipsychotic medication; gabapentin, an anticonvulsant drug used to treat seizure disorders; the anxiety drug lorazepam and the antihistamine diphenhydramine were all present in Smith’s system when she died.

McGeehan’s toxicology report listed the same drugs, as well as eszopiclone or zopiclone, a prescription sedative; the antidepressant drug known as citalopram or escitalopram; desmethyl loperamide, a chemical breakdown caused by the anti-diarrhea medication loperamide and cannabinoids.

Caffeine was also presumably detected in both toxicology reports.

After ingesting the meds, McGeehan fatally shot her daughter in the head before turning the gun on herself.

The gunshot wounds – not the substances – were listed as the cause of death in both relatives.

First responders found the duo’s dead bodies laid side-by-side on one of the hotel room’s two beds, according to the investigative report, which said that a semi-automatic pistol was found on top of McGeehan’s chest.

Smith was wearing PJs, with her body tucked underneath a blanket, while a pillow with a bullet hole through it was laid on top of her head, the report disturbingly revealed.

Another note authored by McGeehan stated that Smith had been “very medicated” leading up to her death and that “she slept cuddled with” her twisted mother.

Smith’s phone had also been intentionally left open so that the youngster’s heart-wrenching photographs and videos could be accessible, McGeehan said in the note, according to the report.

Among luggage, a food bag and other miscellaneous items inside the room, cops recovered multiple prescription bottles bearing McGeehan’s name, “an opened package” of THC gummies, multiple broken or empty gabapentin capsules and clear cup with white residue in it.

The disgraced mother apparently committed the unthinkable acts because she was “severely depressed over breakups and on-going custody battle with her ex-husband,” the report claims.

McGeehan’s family also told investigators she had “a history of severe depression” previously treated by prescription medication, as well as “past suicidal ideations,” the docs state.

McGeehan had been embroiled in a bitter custody dispute with her ex-husband, Brad Smith, for roughly nine years after they divorced in 2015, according to court docs and records previously reported by The Post.

She appeared to retain much of the control over her daughter, the 2024 documents showed.

Smith was a cheerleader for Xtreme Cheer, a team that was left heartbroken by her death.

“No words do the situation justice. She was so beyond loved, and she will always be a part of the UXC family,” the organization said in a statement.
